Description

A multidisciplinary team consisting of project managers, physicians, nurses, pharmacists, clinical informaticists, and software developers was designed and implemented an action plan creation tool that can be modified for specific diseases and integrates into the existing EHR. The project includes clinician-facing administrative software, and an EHR embedded, patient-specific disease specific action plan tool. This platform will allow broad based adoption and deployment of action plans for a variety of diseases.
