In 2016, the Office of National Coordinator for Health Information Technology funded real-world pilots to support national interoperability. Through this funding, the Heartland Region piloted the Patient Centered Data Home initiative of interconnected Health Information Exchange system for notification and data sharing. The objective of this study was to characterize the Patient Centered Data Home initiative through Indiana Health Information Exchange’s participation in the Heartland Region Pilot.

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: Attendees will be able to identify the role of existing infrastructure, such as Health Information Exchanges, in reaching national interoperability; define the Patient-Centered Data Home (PCDH) process; describe the goals of the PCDH process; and explain how data from a nationwide network of HIEs can support research and clinical practice.
