Description

Discharge medication reconciliation is a complex, error prone process that can result in medication errors, adverse drug events and hospital readmission. Using our enterprise data warehouse we successfully identified late instances of discharge medication reconciliation occurring after a discharge order was placed. Initial chart review indicates most of these instances reflect near miss medication errors, which may be a harbinger for errors that are not identified and could also guide future process improvement efforts.
