Background: Increasingly, medical/surgical nurses are facing care challenges that present when behavioral and psychosocial problems complicate care of the acutely ill patient. While the electronic health record (EHR) sometimes capture this information in semi-structured, narrative notes and nursing care plans, there is little known as to the scope of these problems in tertiary care settings.

Objective: To visualize the relative frequency of SDOH represented in nursing care plans across different acute care populations, at a large academic teaching hospital.

Methods: At this site, the Clinical Care Classification (CCC) system TM was used. We extracted nursing diagnoses from the EHR that represented social and behavioral problems and compared frequency across clinical units.

Results: Preliminary results show a significant impact on the non-ICU medicine units, particularly in the areas of substance abuse and violent behaviors. Additional study results are pending and will be completed in time for the CIC conference.
