Description
Access to longitudinal electronic notes supports high quality care. Having too many notes could increase the work of manual chart review. We looked at de-identified metadata on notes to examine the burden of excess notes over time. Overall we found no increase in note number per day but noted a high burden of patient notes in patients with frequent hospitalizations.
