Description

Leveraging the electronic health record (EHR) to track ongoing medication adherence, safety and efficacy monitoring, and medical visits of patients prescribed high-cost, high-risk medication can improve patient safety and promote ongoing high-value pharmaceutical care. Safety and efficacy monitoring recommendations for twenty-five high-risk, high-cost medications were incorporated into an EHR-based clinical dashboard. Pharmacists reviewed the dashboard weekly, which led to frequent recommendations (e.g., discontinuation of therapy, referral to palliative care, additional labs/safety assessments, medication adherence education).

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: Participants will be able to describe process for developing a clinical dashboard to monitor high-risk, high-cost medications, understand the role of the clinical pharmacist in population health medication monitoring, and describe formulary management techniques for managing non-formulary medications within a managed care environment.
