We analyzed the results of one-year use of a completely automated outreach system that promotes guideline adherence for the entire patient population across multiple preventive and chronic conditions. Overall adherence rate to guidelines was significantly higher for patient who received the reminder (33.3%) compared to those who did not receive outreach communication (15.6%), providing evidence that a single low cost automated outreach system is effective in increasing patient adherence across multiple conditions

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: Health reminders have previously been proven effective to increase patient adherence to clinical guidelines. However, most studies have examined short-term effects and have focused on a single screening or condition, and many of those interventions are complex and resource intensive and, consequently, not sustainable over time. This retrospective study provides evidence that a single, provider-configurable, automated patient outreach system can be a cost-effective alternative to traditional approaches in increasing guideline adherence for the entire patient population across multiple preventive and chronic conditions.
