Staff and clinicians in a multi-specialty pediatric outpatient facility were unable to locate and track patients who had multiple same-day appointments. An inter-professional team was formed with expertise and representation across all steps in the patient journey. The team utilized an iterative creative process to customize a visual management tool in the EHR which improved communication and patient flow. This successful inter-professional team provides a model for continued collaborative improvement initiatives involving the EHR.

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: The participant will learn 1) to assemble a successful interprofessional team to use information technology to operationalize best practices, and 2) to design an EHR visual management tool to improve communication, patient tracking, and workflow in a multi-specialty outpatient facility.
