Description
We implemented an easily-accessible intervention embedded within the EHR to deliver community and healthcare resources to underserved populations, accessible in as few as three clicks. The impact was studied over a three-month period using provider surveys, patient surveys, and intervention utilization data. Attendees of the presentation will gain the ability to implement a practical means of delivery of locally-relevant community-based resources.
