Description

Sickle cell patients are one of the largest utilizers of health care services. Sickle cell has the highest observed 30 day readmission rate for both inpatient hospital stays and emergency room encounters. In this paper, we discuss our experience developing and optimizing a care path and analytics platform for sickle cell anemia. This program focuses on key process and clinical measures designed to reduce cost and improve patient outcomes.
